It is our goal to make our services and assistive products available to all people. To that end, we can accept most insurance coverage. A listing of insurance companies we currently bill is provided to the left. If you have coverage with another company and would like to receive services from us, please call us and let us know. We will do our best to accomodate your needs.

Before we begin your appointment, we will discuss with you what services or products may be covered by your insurance and which may not. It is important to us that you know what your responsibilities will be and how far your insurance will serve you. Listed below are some helpful suggestions to aid you in using your insurance coverage for our services and products and make your appointment as easy as possible.

Things To Remember When Scheduling An Appointment
When scheduling an appointment, being prepared can help us serve you better and help make your appointment go more quickly. You can download our patient information form and bring that to your appointment to save time. The patient information form may be downloaded from our online scheduling page. The following is a list of things that we may ask you about when you call.

  • If you are a new patient, we would like to get your insurance information before you arrive to our clinic. This helps the process to go much more quickly as we can prepare much of the information before your appointment. We will usually ask for the following:
        - Insurance Company and plan
        - Your Social Security Number (for verification only - this is not mandatory)
        - Your policy and group number
        - The primary insured, if your insurance is through another person
        - Your primary care physician and address
  • Many services we provide require a referral from your primary care physician. If you have already received a referral from your doctor, it will be very helpful to receive the referral before your appointment.

Tips for Your Appointment
There are a number of things that you can do to make your appointment go more quickly and easier. The following list contains a few of the things that we have found helpful.

  • You will need both your insurance card and your Minnesota Driver's license or ID card.
  • You should always bring all your health insurance cards and information. We may need copies of secondary insurance cards, supplemental insurance cards, or other additional insurance information.
  • We will always attempt to inform you for which services and/or products your insurance coverage will pay. However, it is always the patient's responsibility to know your insurance coverage. It is always helpful to review the information provided by your insurance company to be sure of the things they will and won't pay.
  • You should always be prepared to show your insurance card(s) when checking in for any appointment. Your information will be checked each visit to ensure it is current. We will periodically need to make additional copies of your insurance card(s), so you should always have them with you.
